A confidential Student Medical Form is required for every applicant. After you submit the application, the medical-form button will become available near the bottom of this page. The medical form must also be completed as part of the application process.

I understand that it is my responsibility to ensure that I inform the Marketing & Public Relations staff on the day of the event if I do not wish to appear in photographs/videos.
Please note this can only relate to individual photos and not if the child is the member of school ensembles.
I understand that appearing in photographs and/or videos while performing in groups in unpreventable.

All images will be stored in a secure location and only authorized staff will be able to access them.
